You agree that you are entering eyeglass prescription information that is accurate. We will verify that your name matches the name on the prescription and will produce lenses according to the information you enter. We take no responsibility for incorrectly entered information.

Prescription lenses go into immediate production and may not be cancelled or refunded once they are made.
